Local information template

This local information template has been developed to provide a guide to the types of information that people affected by neurological conditions told us they wanted at a local level. The distribution and availability of resources and support for people with neurological conditions is very different from area to area so it is important for local stakeholders to identify, map and gather together the local picture. This template is provided as a tool for local stakeholders to start the process of developing a local information resource (or to provide guidance on what should be built into an existing resource).

It is also important to gather information on referral mechanisms and criteria, if any, to sources of support as these vary from area to area.

We have provided a guide which suggests possible local sources for information relating to the various headings in the template. You won’t find everything we suggest in every area, but the aim should be to locate and document the nearest source of information and support on any particular issue and to fill in the whole template as far as possible.

Local stakeholders will often be the best source of information on what is available locally and what they have found useful. In particular, it would be advisable to invite local groups of people affected by neurological conditions e.g. MS Society branches, Parkinson’s disease society branches to contribute to the completion of the Local Information Template.
